wellness journey. Your medical records and treatment history with FNP Jacobs will remain
available to ensure continuity of care.
If you wish to continue with your current care plan, no action is needed. Your upcoming
appointment will be seamlessly reassigned to another qualified provider within
approximately 45 minutes of your original appointment time.
